Job Summary

This position provides supervision and resource support to the assigned Clinical Alignment team, assures effective transitions of care to the post-acute environment, and maintains relationships with hospital administration and referral sources. Assists branch operations in clinical coordination and communication with referral sources. Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordinate the necessary resources.

Essential Functions

  • Serves as a resource for Clinical Alignment Team. Hires, leads, mentors, develops, and evaluates staff in a manner that promotes engagement, productivity, and ensures competencies. Mentors staff to increase clinical skills, critical thinking, problem solving skills and promote retention.

  • Conducts individual and team goal setting, performance planning and evaluation. Participates in strategic planning, management meetings and leaderships training opportunities.

  • Develops and maintains contact with key hospital, skilled nursing, assisted living facilities, case management and hospital clinical staff to provide ongoing updates on Banner Homecare and Hospice services available in the market. Facilitates communication between referral sources and key stake holders and Banner Homecare and Hospice.

  • Supports change and participates in the development, interpretation, implementation and evaluation of process improvement and quality management activities. Works with staff to relay and implement necessary changes.

  • Provides departmental leadership by demonstrating effective communication skills, problem solving, and proactive conflict resolution to internal and external customers in situations.

  • May need to function in role of RN Clinical Alignment Specialist in the event of a staffing gap.

Minimum Qualifications

Requires a current, valid RN license in state of practice. BSN preferred.
